What to wear
Comfortable clothes that allow you to move and allow your physical therapist to access and observe the treatment area. A tank top if the area to be treated is your neck, upper back or shoulder. Shorts if the area to be treated is your lower back, hip, knee or ankle.
What to bring
Your insurance card and ID to your first appointment, applicable claim number for Labor & Industries or PIP (auto accident).
It is highly recommended that you contact your insurance provider prior to your first appointment so that you fully understand your physical therapy benefits.
Copay due at time of service. We accept cash, check, MC, Visa, Discover, AmEx, Venmo, Apple Pay and Android Pay.
